Services
1. Employers submit draft employment contracts to the Embassy for approval via email at thaiconsularwarsaw@thaiemb.pl. The subject of the email should indicate clearly the name of the company, while the body of the email should provide a list of prospective employees (with names and numbers of passports). Documents to be submitted are:
1.1 Draft employment contracts in two languages (English and Thai). These draft contracts should NOT be affixed with the employers’ and employees’ signatures.
1.2 Copies of passports of the employers and employees.
2. The Embassy will inform the employers of the result approximately within seven (7) working days (starting after the date of submission) via email.
3. The employer has to verify his/her signature on the English version of the contract officially by the Polish Chamber of Commerce in Warsaw (Krajowa Izba Gospodarcza)/regional chambers of commerce 1 (or by the Notary public & the president of the competent regional court (sąd okręgowy) 2), and then legalize it at the Ministry of Foreign Affairs of the Republic of Poland.
4. Employers submit the employment contract, legalized by the relevant local authorities, to the Consular Section of the Embassy with supporting documents as follow:
4.1 Legalization Request Form (please request a form via thaiconsularwarsaw@thaiemb.pl) and 80 PLN fee (cash only) per contract;
4.2 A copy of passport of the employer (with the employer's signature and date);
4.3 A copy of passport of the employee (with the employee's signature and date);
4.4 A copy of Permit for Employment;
4.5 A copy of every page of the Thai version of the employment contract;
4.6 A copy of every page of English version of the employment contract (along with a page containing the legalization stamp of the Ministry of Foreign Affairs in Warsaw);
4.7 In case of new salon, photos of the salon and accommodation for the employee are required.
If all required documents are in order, the Embassy will consider and approve the contracts approximately within three (3) working days (starting after the date of submission). The Embassy will only certify the genuine signature of the authorized person of relevant local authorities and will assume no responsibility for the content of the document. This is in accordance with the Regulations of the Ministry of Foreign Affairs on Legalization of Documents B.E. 2539 (1996).
5. Once the employment contracts are legalized by the Embassy, the employers should send the said contracts with the Embassy’s seal to Thailand for the employees to sign. Afterwards, the Thais should report their travelling abroad to the provincial office of employment, the district 1-10 office of employment, or the Office of Overseas Employment Administration (additional information on this can be found at http://www.overseas.doe.go.th/reEntry.php).
IMPORTANT INFORMATION:
1 In the case that the certification of signature on the employment contract is made by the Polish Chamber of Commerce in Warsaw (Krajowa Izba Gospodarcza)/regional chambers of commerce, a sworn translation of the aforementioned certification is not required.
2 In the case that the notarial certification of signature on the employment contract is made by the Notary public in Polish language, the Employer is obliged to prepare a sworn translation of the aforementioned certification into English and submit it together with all required documents.

Documents intended for circulation in Thailand must be prior-legalized by the Consular Department of the Ministry of Foreign Affairs, Poland.

Documents certified by the Ministry of Foreign Affairs must be submitted for legalization at the Consular Section, Royal Thai Embassy, Willowa 7, 00-790 Warsaw from Monday to Friday between 09.00 - 12.00 hours. Documents must be submitted in person. We are not able to accept documents send by post/courier company. Additionally, we would like to inform that in case that you are not able to submit documents personally you may authorize other person or consular services company to submit documents on your behalf. The processing time for legalization is 2-3 business days.

Please kindly visit Police Clearance Service Center official website: https://pcscenter.sbpolice.go.th/en to see the instruction and a list of required documents to obtain Thai Police Clearance Certificate. One of the requirement is to send two sets of original fingerprints to Thailand. If applicants are not able to take fingerprints at local police station, please process it at the Royal Thai Embassy in Warsaw. Applicants to submit documents and obtain fingerprints at the Royal Thai Embassy in Warsaw must make an appointment at the following link https://thaiembassywarsaw.setmore.com/. Consular Section is open on Monday to Friday from 9.30 – 12.00. Strictly no walk-in applicants are allowed.

บุคคลสัญชาติไทยที่มีบุตรเกิดในประเทศโปแลนด์ สามารถแจ้งเกิดบุตรที่สถานเอกอัครราชทูตฯเพื่อขอ สูติบัตรไทย ให้บุตรได้ และหากบุตรของท่านเป็นเด็กลูกครึ่งสัญชาติไทยและสัญชาติโปแลนด์ หรือสัญชาติอื่นๆ บุตรของท่านสามารถถือสัญชาติไทยควบกับสัญชาติของบิดาหรือมารดาต่างด้าวได้ ไปจนถึงอายุ 20 ปีบริบูรณ์
เอกสารและหลักฐานที่จะต้องยื่นเพื่อขอจดทะเบียนสูติบัตร มีดังนี้
1. สูติบัตรโปแลนด์ (แปลเป็นภาษาอังกฤษ และรับรองโดยกระทรวงการต่างประเทศของโปแลนด์)
2. ใบสำคัญการสมรสของบิดาและมารดา หรือ คำพิพากษาหย่าที่มีผลบังคับใช้แล้ว หรือ หนังสือรับรองบุตร (ภาษาอังกฤษหรือไทย)
3. บัตรประจำตัวประชาชนและหนังสือเดินทางของบิดาและมารดาที่ยังมีอายุใช้งานอยู่
4. สำเนาทะเบียนบ้านไทยของบิดาหรือมารดา (กรณีคนไทย)
5. ใบเปลี่ยนชื่อหรือนามสกุล (กรณีบิดา มารดา หรือเด็กเคยเปลี่ยนชื่อหรือนามสกุล)
6. รูปถ่ายหน้าเด็ก (ถ่ายตรง)
7. ใบแสดงถิ่นที่อยู่ของบิดา-มารดาต่างชาติ (ภาษาอังกฤษ) สำหรับของโปแลนด์จะเรียกว่า zaświadczenia o zameldowaniu

• การแจ้งเกิดตามกฎหมายไทยที่สถานเอกอัครราชทูตฯ ไม่เสียค่าธรรมเนียม
• หากประสงค์จะแจ้งเกิดบุตร 2 คนหรือมากกว่า กรุณาจัดเตรียมเอกสารต่างๆ เพิ่มตามจำนวนบุตรด้วย
• ทำสูติบัตรที่สถานเอกอัครราชทูตฯ ใช้เวลาประมาณ 1-2 ชม
• บิดา มารดา และบุตรจะต้องมาที่สถานทูตเพื่อทำสูติบัตร
หมายเหตุ หลังจากแจ้งเกิดบุตร ที่สถานเอกอัครราชทูตฯ แล้ว ควรทำหนังสือเดินทางไทยให้บุตรที่สถานเอกอัครราชทูตฯ และ เพิ่มชื่อบุตรเข้าในทะเบียนบ้านไทย หากไม่ทำการเพิ่มชื่อบุตรเข้าทะเบียนบ้านไทย บุตรจะเสียสิทธิในประเทศไทย เช่น บุตรอาจจะไม่สามารถเข้าโรงเรียนในประเทศไทยได้
การแจ้งชื่อบุตรเข้าทะเบียนบ้านไทย
** สถานเอกอัครราชทูตฯ ไม่สามารถ แจ้งอำเภอไทย เพื่อเพิ่มชื่อเด็กเข้าทะเบียนบ้านไทยให้ได้ ท่านต้องไปแจ้งที่อำเภอไทย ซึ่งโดยทั่วไปต้องไปติดต่อด้วยตนเองเนื่องจากต้องแสดงหนังสือเดินทางไทยของเด็กที่ได้รับการประทับตราเข้าเมืองจากเจ้าหน้าที่ตรวจคนเข้าเมืองไทยไปแสดง อย่างไรก็ตาม ควรสอบถามจากอำเภอ/เขตโดยตรง **
การเพิ่มชื่อบุตรเข้าทะเบียนบ้านไทย
• นำหนังสือเดินทางไทยของบุตรที่ได้รับการประทับตราเข้าประเทศไทยจากเจ้าหน้าที่ตรวจคนเข้าเมืองของไทย และใบเกิดบุตร (สูติบัตรไทย) ที่ออกโดยสถานเอกอัครราชทูตฯไปแจ้งชื่อบุตรเข้าทะเบียนบ้านที่อำเภอที่ท่านมีชื่ออยู่ใน ทะเบียนบ้าน [หากไม่ได้ทำหนังสือเดินทางไทยให้บุตรที่สถานเอกอัครราชทูตฯ และเดินทางเข้าประเทศไทยโดยใช้หนังสือเดินทางของประเทศอื่น หรือใบสำคัญประจำตัว (ใบซี.ไอ.) ก็สามารถเพิ่มชื่อเด็กเข้าทะเบียนบ้านไทยได้ แต่ต้องไปติดต่อสำนักงานตรวจคนเข้าเมืองของไทยเพื่อทำการพิสูจน์สัญชาติเด็กก่อน]
• เอกสารประจำตัวบิดา/มารดาที่มีสัญชาติไทย เช่น บัตรประจำตัวประชาชน ทะเบียนบ้าน ทะเบียนสมรส
• คัดสำเนาทะเบียนบ้านไทยของบุตร เพื่อนำมาใช้ที่ประเทศโปแลนด์

7. ใบแสดงถิ่นที่อยู่ของบิดา-มารดาต่างชาติ (ภาษาอังกฤษ) สำหรับของโปแลนด์จะเรียกว่า zaświadczenia o zameldowaniu

**เจ้าหน้าที่อาจเรียกเอกสารเพิ่มเติมได้แล้วแต่กรณี ***
• การแจ้งเกิดตามกฎหมายไทยที่สถานเอกอัครราชทูตฯ ไม่เสียค่าธรรมเนียม
• หากประสงค์จะแจ้งเกิดบุตร 2 คนหรือมากกว่า กรุณาจัดเตรียมเอกสารต่างๆ เพิ่มตามจำนวนบุตรด้วย
• ทำสูติบัตรที่สถานเอกอัครราชทูตฯ ใช้เวลาประมาณ 1-2 ชม
• บิดา มารดา และบุตรจะต้องมาที่สถานทูตเพื่อทำสูติบัตร
หมายเหตุ หลังจากแจ้งเกิดบุตร ที่สถานเอกอัครราชทูตฯ แล้ว ควรทำหนังสือเดินทางไทยให้บุตรที่สถานเอกอัครราชทูตฯ และ เพิ่มชื่อบุตรเข้าในทะเบียนบ้านไทย หากไม่ทำการเพิ่มชื่อบุตรเข้าทะเบียนบ้านไทย บุตรจะเสียสิทธิในประเทศไทย เช่น บุตรอาจจะไม่สามารถเข้าโรงเรียนในประเทศไทยได้
การแจ้งชื่อบุตรเข้าทะเบียนบ้านไทย
** สถานเอกอัครราชทูตฯ ไม่สามารถ แจ้งอำเภอไทย เพื่อเพิ่มชื่อเด็กเข้าทะเบียนบ้านไทยให้ได้ ท่านต้องไปแจ้งที่อำเภอไทย ซึ่งโดยทั่วไปต้องไปติดต่อด้วยตนเองเนื่องจากต้องแสดงหนังสือเดินทางไทยของเด็กที่ได้รับการประทับตราเข้าเมืองจากเจ้าหน้าที่ตรวจคนเข้าเมืองไทยไปแสดง อย่างไรก็ตาม ควรสอบถามจากอำเภอ/เขตโดยตรง **
การเพิ่มชื่อบุตรเข้าทะเบียนบ้านไทย
• นำหนังสือเดินทางไทยของบุตรที่ได้รับการประทับตราเข้าประเทศไทยจากเจ้าหน้าที่ตรวจคนเข้าเมืองของไทย และใบเกิดบุตร (สูติบัตรไทย) ที่ออกโดยสถานเอกอัครราชทูตฯไปแจ้งชื่อบุตรเข้าทะเบียนบ้านที่อำเภอที่ท่านมีชื่ออยู่ใน ทะเบียนบ้าน [หากไม่ได้ทำหนังสือเดินทางไทยให้บุตรที่สถานเอกอัครราชทูตฯ และเดินทางเข้าประเทศไทยโดยใช้หนังสือเดินทางของประเทศอื่น หรือใบสำคัญประจำตัว (ใบซี.ไอ.) ก็สามารถเพิ่มชื่อเด็กเข้าทะเบียนบ้านไทยได้ แต่ต้องไปติดต่อสำนักงานตรวจคนเข้าเมืองของไทยเพื่อทำการพิสูจน์สัญชาติเด็กก่อน]
• เอกสารประจำตัวบิดา/มารดาที่มีสัญชาติไทย เช่น บัตรประจำตัวประชาชน ทะเบียนบ้าน ทะเบียนสมรส
• คัดสำเนาทะเบียนบ้านไทยของบุตร เพื่อนำมาใช้ที่ประเทศโปแลนด์

การบริการต่ออายุบัตรประจำตัวประชาชนไทย
ผู้ยื่นคำร้องต้องเป็นผู้ที่เคยมีบัตรประจำตัวประชาชนไทยมาก่อนแล้วเท่านั้น และต้องมีถิ่นพำนักอยู่ในโปแลนด์ ตามรายละเอียดดังต่อไปนี้
1. หลักเกณฑ์ในการให้บริการ
1.1 บัตรเก่าหมดอายุ
1.2 บัตรสูญหาย ชำรุด (รวมถึงการแก้ไขคำผิด และการเปลี่ยนแปลงชื่อหรือนามสกุล)
2. หลักฐานที่ต้องใช้ประกอบ
2.1 บัตรประจำตัวประชาชนไทยที่มีอยู่ (ใช้ในกรณีต่ออายุบัตร ยกเว้นกรณีบัตรหาย)
2.2 หนังสือเดินทางไทย (ใช้ในทุกกรณี)
2.3 ใบแจ้งความเป็นจากสถานีตำรวจโปแลนด์ (กรณีบัตรหาย)
2.4 ค่าธรรมเนียม 20 สว้อตตี้ (กรณีบัตรหาย/ขอเปลี่ยนบัตรใหม่เนื่องจากชำรุด) (กรุณาเตรียมเงินให้พอดี ใช้ในทุกกรณี)
2.5 เอกสารของราชการไทยที่แสดงการเปลี่ยนแปลงรายละเอียดที่ขอแก้ไข เช่น ใบเปลี่ยนชื่อหรือนามสกุล เป็นต้น (เฉพาะการเปลี่ยนแปลงรายละเอียดในบัตร)
2.6 บัตรหายหรือชำรุดในบางกรณีอาจต้องนำพยานบุคคลอ้างอิงที่น่าเชื่อถือได้มาแสดงตัวเป็นผู้รับรองในวันที่รับบริการ โดยพยานบุคคลจะต้องนำหนังสือเดินทางไทยและบัตรประจำตัวประชาชนไทยมาแสดงด้วย
อนึ่ง กรณีการขอมีบัตรประจำตัวครั้งแรกยังคงต้องยื่นคำร้องขอรับบริการในประเทศไทย

การขอหนังสือเดินทางอิเล็กทรอนิกส์เล่มแรก (หนังสือเดินทางเล่มแรกมีอายุ 5 ปี)
เด็กที่มีสิทธิยื่นคำร้องขอหนังสือเดินทางไทย
• เด็กที่มีสัญชาติไทย โดยมีบิดามารดาฝ่ายใดฝ่ายหนึ่ง หรือทั้งสองฝ่ายเป็นผู้มีสัญชาติไทย และมีสูติบัตรไทยแล้ว
• เนื่องจากการออกหนังสือเดินทางอิเล็กทรอนิกส์มีการเก็บข้อมูลชีวภาพ ได้แก่ ลายนิ้วมือและภาพถ่ายของผู้ร้องฯ ดังนั้น เด็กจะต้องมายื่นคำร้องขอหนังสือเดินทางด้วยตนเอง พร้อมบิดาและมารดา
• หากเด็กยังไม่มีสูติบัตรไทย ต้องดำเนินการแจ้งเกิดก่อน

• บิดาและมารดา หรือ ผู้มีอำนาจปกครองลงนามให้ความยินยอมในแบบคำร้อง
• ค่าธรรมเนียม 160 สว้อตตี้ ( กรุณาเตรียมเงินมาให้พอดี)
หมายเหตุ
o บิดามารดาจะต้องมาลงนามให้ความยินยอมในการออกหนังสือเดินทางต่อหน้าเจ้าหน้าที่ หากบิดาและมารดาไม่สามารถมาลงนามต่อหน้าเจ้าหน้าที่ได้ จะต้องไปลงนามใน “ หนังสือมอบอำนาจและให้ความยินยอม” (สำหรับการทำหนังสือเดินทางให้ผู้เยาว์) ต่อหน้าเจ้าหน้าที่ Notary Public เพื่อรับรองรายมือชื่อของบิดาและมารดา
o ในกรณีที่บิดาและมารดาอยู่ที่ประเทศไทย ทั้งบิดาและมารดาต้องทำ “หนังสือให้ความยินยอม” ให้บุตรทำหนังสือเดินทาง ผ่านกรมการกงสุล กระทรวงการต่างประเทศ [กรุงเทพฯ] หรือผ่านที่ว่าการอำเภอ [ต่างจังหวัด]
o กรณีที่บิดามารดาจดทะเบียนหย่าแล้ว ให้ฝ่ายที่มีอำนาจปกครองบุตรเป็นผู้ลงนามในหนังสือยินยอมเพียงฝ่ายเดียวและจะต้องแสดงหลักฐานทะเบียนหย่า หรือคำสั่งศาลซึ่งระบุว่าเด็กอยู่ในความปกครองของบิดา/มารดาหรือผู้มีอำนาจปกครอง
o กรณีบิดาหรือมารดาของผู้เยาว์เสียชีวิต จะต้องแสดงหลักฐานใบมรณบัตร
เอกสารเพิ่มเติมตามความจำเป็นแต่ละกรณี
1. ใบเปลี่ยนชื่อ/ชื่อสกุล
– กรณีใช้ชื่อ/ชื่อสกุลใหม่ที่ไม่ตรงกับหนังสือเดินทางฉบับเดิมหรือรายการแก้ไขหนังสือเดินทางที่ปรากฏในหนังสือเดินทางฉบับเดิม
2. ใบสำคัญการสมรส
– สตรีที่สมรสแล้วขอเปลี่ยนชื่อสกุล
– มารดาที่มีชื่อสกุลไม่ตรงกับสูติบัตรของบุตร
3. ใบสำคัญการหย่า
– สตรีที่ใช้ชื่อสกุลไม่ตรงกับหนังสือเดินทางฉบับเดิมเนื่องจากหย่า
4. บันทึกการหย่า
– ใช้ประกอบการแสดงอำนาจในการปกครองบุตร
5. หลักฐานการจดทะเบียนรับรองบุตร
– กรณีผู้เยาว์ใช้ชื่อสกุลของบิดา โดยที่บิดา/มารดาไม่ได้จดทะเบียนสมรส (ใบปกครองบุตรเพียงฝ่ายเดียว ปค.14)
6. ใบสำคัญประจำตัวคนต่างด้าวของบิดา/มารดา
– ผู้เยาว์ที่บิดา/มารดาเป็นคนต่างด้าว
หากผู้ร้องฯ ไม่สามารถแสดงเอกสารอย่างใดอย่างหนึ่งข้างต้น ขอให้แจ้งเหตุขัดข้องที่ไม่สามารถแสดงเอกสารดังกล่าวฯ พร้อมทั้งแสดงเอกสารอย่างอื่นที่แสดงความเป็นคนไทย เช่น ใบอนุญาตขับขี่รถยนต์ของไทย โดยสถานเอกอัครราชทูตฯ จะพิจารณาอนุโลมเป็นรายกรณี
หากประสงค์ให้สถานเอกอัครราชทูตฯ จัดส่งกลับให้ทางไปรษณีย์ ต้องชำระค่าส่ง 30 สว้อตตี้
หมายเหตุ สถานเอกอัครราชทูตฯ ไม่รับผิดชอบกรณีเอกสารที่ส่งทางไปรษณีย์ชำรุดหรือสูญหาย ไม่ว่ากรณีใด

– สตรีที่ใช้ชื่อสกุลไม่ตรงกับหนังสือเดินทางฉบับเดิมเนื่องจากหย่า
4. บันทึกการหย่า
– ใช้ประกอบการแสดงอำนาจในการปกครองบุตร
5. หลักฐานการจดทะเบียนรับรองบุตร
– กรณีผู้เยาว์ใช้ชื่อสกุลของบิดา โดยที่บิดา/มารดาไม่ได้จดทะเบียนสมรส (ใบปกครองบุตรเพียงฝ่ายเดียว ปค.14)
6. ใบสำคัญประจำตัวคนต่างด้าวของบิดา/มารดา
– ผู้เยาว์ที่บิดา/มารดาเป็นคนต่างด้าว
หากผู้ร้องฯ ไม่สามารถแสดงเอกสารอย่างใดอย่างหนึ่งข้างต้น ขอให้แจ้งเหตุขัดข้องที่ไม่สามารถแสดงเอกสารดังกล่าวฯ พร้อมทั้งแสดงเอกสารอย่างอื่นที่แสดงความเป็นคนไทย เช่น ใบอนุญาตขับขี่รถยนต์ของไทย โดยสถานเอกอัครราชทูตฯ จะพิจารณาอนุโลมเป็นรายกรณี
